Diabetes Health Partnership Mission
Our mission is to identify the person at risk for developing diabetes and the patient with existing diabetes. We will provide health care and education personalized to each individual that will optimize management and control of diabetes. We will work with our patients as a team to help them achieve a balanced healthy diabetic lifestyle.
Diabetes is a disease in which the body does not produce or properly use Insulin. In diabetes the blood glucose (sugar) level is above normal. The cause of diabetes is still unknown. However, both genetics and environmental factors such as obesity and lack of exercise appear to play a role.
Risk Factors for diabetes include:
- Over 40 years of age
- Overweight
- Family history of diabetes
- High blood pressure
- Elevated triglycerides or elevated LDL
- Inactivity
- Have had diabetes during pregnancy
- Pre-diabetes
Symptoms of Type 2 diabetes are easily overlooked as they build up gradually. They may include
- Fatigue
- Frequent infections
- Frequent urination
- Blurred eyesight
- Dry, itchy skin
- Numbness or tingling in hands or feet
- Slow healing cuts or sores
- Increased hunger
- Increased thirst
- Problems with sexual function
