Social Worker (LMSW or LCSW)

    As a Social Worker at Cascade Medical Center you will work in a variety of settings. The Social worker will work in collaboration with other Social Workers and healthcare team members to provide Behavioral Health interventions in the family practice, Social Work interventions in the Hospital/ER and will work independently to provide Mental Health Counseling for individuals, families and groups.

    As a Mental Health Counselor, you will conduct psychosocial assessments, provides diagnosis, treatment planning, education and therapeutic interventions for patients dealing with mental health and substance use disorders, family discord and other life stressors. You will provide individual, family and group counseling as need presents.

    As an Integrated Behavioral Health Consultant, you will consult with medical providers on their patients. You will conduct Health and Behavior Assessments and provide brief behavioral health interventions for patients with chronic illnesses who's behaviors negatively impact their disease management or health outcomes.

    As a Medical Social Worker, you will provide advocacy, education, safety planning, assessments and interventions to the patients and families in the hospital setting. While coordinating with medical and nursing staff and outside organizations, you will develop specific treatment plans and help facilitate transition planning that provide for the best possible outcomes for our patients and their families.
